Southeast Georgia Health System Welcomes Two New Nurse Practitioners to Primary Care

Southeast Georgia Health System is pleased to welcome Victoria “Tori” Wolfes, FNP-C, and Christopher Heath Brown, FNP-C to Southeast Georgia Physician Associates-Primary Care. Both providers bring a shared commitment to delivering high-quality, patient-centered care to the communities we serve.
Tori Wolfes, FNP-C
Wolfes is a certified family nurse practitioner with nearly a decade of nursing experience in high-acuity environments, including emergency departments, intensive care units and critical care resource teams. This background equips her to manage a broad range of health needs commonly encountered in primary care.
She earned both her Bachelor of Science in Nursing and Master of Science in Nursing from Georgia Southwestern State University in Americus, Georgia. Her extensive clinical training includes care for adult, pediatric, women’s health and geriatric populations. Most recently, she practiced at Advanced Pediatric Care in Jacksonville, Florida, where she honed her skills in wellness exams, acute visits and chronic disease management.
Known for her clear communication, attention to detail and commitment to personalized care, Wolfes is dedicated to helping patients achieve lasting health and wellness. Her ability to coordinate care across specialties and navigate complex medical needs makes her a valuable asset to the team.
Wolfes is accepting new patients at Southeast Georgia Physician Associates–Primary Care, located at 5000 Wellness Way, St. Simons Island.
Christopher Heath Brown, FNP-C
With over 13 years of nursing experience and a deep understanding of comprehensive patient care, Brown brings a dynamic, results-driven approach to the primary care team. Most recently, he served patients at Coastal Community Health Services in Brunswick, focusing on continuity of care, patient education and chronic disease management.
His career spans emergency care, cardiac stepdown units, hospice, home health and medical-surgical telemetry across Georgia, Tennessee and Hawaii. Known for his patient-focused care and efficient coordination of treatment plans, Brown excels at managing complex health needs while empowering patients and families to take an active role in their care.
Brown earned his Associate of Science in Nursing from Darton College School of Nursing in Albany, Georgia, his Bachelor of Science in Nursing from Columbus State University in Columbus, Georgia and his Master of Science in Nursing from Walden University in Minneapolis, Minnesota.
Brown is accepting new patients at Southeast Georgia Physician Associates–Primary Care located at 3222 Shrine Road in Brunswick.
